Breast augmentation involves surgical procedures aimed at increasing the volume and enhancing the shape of the breasts. It is among the most frequently performed aesthetic surgeries, boasting high patient satisfaction rates. Implants are inserted through incisions made at the inframammary fold, periareolar region, or axilla. The implants are positioned either beneath the pectoral muscle (dual plane) or above it (subfascial/subglandular plane).

Silicone implants are predominantly used, with manufacturers offering lifetime warranties. Currently, fifth-generation implants are commonly utilized. The popularity of saline-filled implants is diminishing.

The decision is based on individual assessments. Generally, patients with thin skin and minimal tissue receive implants in the dual plane position, where the upper part is under the muscle and the lower part under the breast tissue. If sufficient breast tissue exists, subglandular placement is also viable.

Typically, the procedure is performed on individuals over 18 years of age. It can be conducted during any season.

Incisions are made at the inframammary fold, periareolar area, or axilla. In our practice, the inframammary approach is preferred, resulting in a 3–4 cm scar that significantly fades by the sixth postoperative month. Scar-reducing creams are recommended during this period. However, it's important to note that all surgical procedures leave some scarring.

Post-surgery, incisions are covered with bandages, and patients are fitted with a compression bra. Ambulation is encouraged on the first night. Drains are generally not used unless necessary. Pain is uncommon, though those with submuscular implants may experience pressure sensations. Patients are typically discharged the next day. Scar-reducing creams are advised from the third week. Regular follow-ups are scheduled, and some patients may benefit from postoperative massages. Initially, implants may appear elevated, but they settle into a natural position by the third month. Swelling and asymmetry are normal in the early stages, with full adaptation taking up to a year. Strenuous activities should be avoided for the first three weeks, and chest exercises for six weeks. Swimming is permissible after one month. Special bras are recommended for the first six weeks, followed by sports bras for the next six months. Sleeping on the implants should be avoided during the first month.

When implants are placed via the axillary or inframammary routes, the milk ducts remain unaffected, thus not impeding breastfeeding.
